General Information

Kern Legacy Max Choice is a County Self-Insured health plan that provides an employee/retiree a medical plan option with Maximum options and flexibility. This plan offers you an affordable 20% co-insurance and low deductibles that utilizes the same Anthem Blue Cross network of providers as Kern Legacy Classic Choice.

  This document lists all the procedures and services that do not require a Prior Authorization while seeking services through a contracted provider. Non-contracted providers must obtain a Prior Authorization when providing these procedures and services.

Claims Submission

• Provider Portal

  Visit the HealthEdge Administrators website to view processed claims and print EOBs (copies of EOBs will be mailed with checks to providers).

• Electronic Submission

  Claims may be submitted electronically to Anthem Blue Cross. Use Payor ID 47198.

• Paper Claims

  Claims can also be submitted via traditional paper claim to:
